Waiver and Release of Liability

I hereby acknowledge that I have voluntarily chosen to participate in physical activities at SAN-Q Movement Rehab Inc. I understand that these activities may involve strenuous exercise and have inherent risks, including but not limited to the risk of injury, illness, or property damage.

In consideration of being allowed to use the facilities and services provided by SAN-Q Movement Rehab Inc., I hereby release, waive, discharge, and covenant not to sue SAN-Q Movement Rehab Inc., its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me while participating in activities at SAN-Q Movement Rehab Inc.

I understand and agree that this release of liability includes any claims based on negligence, breach of contract, or any other legal theory, and applies to all activities and services provided by SAN-Q Movement Rehab Inc., both on and off the premises.

I further agree to indemnify and hold harmless SAN-Q Movement Rehab Inc., its owners, employees, agents, and representatives from any and all claims or damages, including legal fees, arising out of or related to my use of the facilities and participation in activities at SAN-Q Movement Rehab Inc.

I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to its provisions. I acknowledge that I am signing this document freely and voluntarily, without any inducement or duress.

Cancellation Policy

The San-Q Movement Rehab Studio requires Twenty-four (24) hours notice for any change or cancellation. It will be billed for any session booked if Twenty-four (24) hours notice is not given. For the prepaid packages, there are One emergency cancellations for each 10-class package without being billed. For semi-package, if one client need to cancel the class within Twenty-four (24) hours under the emergency cancellation term, the other client could change this class to private class by paying the difference or invite the friend to group this semi class. And if the other client insists on canceling this class, it will be counted as one emergency cancellation as well. ●All prepaid sessions are fully transferable but not refundable or sharable. No refund will be granted after the said validity periods.
